In-Network Insurance
We are currently accepting: BCBS, UHC, MODA, PacificSource, and Providence insurance plans.
Out-of-Network and Out-of Pocket Options
We do offer superbills for some out-of- network plans.

Fees for standard services include:
60 minute initial evaluation: $400
25 minute follow-up visit: $150

Miscellaneous
The fees below are typically NOT covered by insurance:
​
Phone calls: $25 for 5-10 minutes, $85 for 11-20 minutes, $130 for 21-30 minutes.
​
Written documents: $50 per 15 minutes
